Services

Community PACE provides all medical and social services to the elderly population with chronic care needs to help them maintain their independence in their home and community for as long as possible. At Community PACE, our care team works cohesively with participants and their caregivers to provide the best plan of care for each individual enrolled into our program.

Our services include but are not limited to:

  • Adult day care at the Community PACE Day Center that offers primary care; physical and occupational therapies; daily meals that include breakfast and lunch; nutritional counseling and guidance; social work and personal care
  • Primary care provided by Community PACE providers
  • Specialty care such as Neurology, Cardiology, and Urology
  • Dental, vision and hearing services
  • Necessary medical equipment
  • In home assistance with bathing, light cleaning, and meals when necessary
  • All necessary prescription and OTC drugs
  • Transportation to the Community PACE Day Center and to any specialty care appointments
  • Social services
  • Mental health services
  • Respite care 
  • Hospital and nursing home care when necessary
  • End of life and hospice care
